I wish to exercise my right of appeal under the School Standards & Framework Act 1998. I am the person with legal parental responsibility for the child named above and the information given is true to the best of my knowledge and belief. I understand that if I do not attend the hearing, the appeal will be heard in my absence using the information I have supplied on this form together with any other information sent to the Clerk to the Appeal Panel before my hearing date. I agree for my data to be stored electronically for two years after the appeal hearing and to be contacted by post, e mail, text and telephone. I agree that the Principal Clerk shall be sent by my Local Authority a copy of my original application form or a 'screenshot', together with name of the school allocated. I confirm I have listed the names of any friends, witnesses or representatives and their capacity who will be attending the hearing. I agree for copies of paperwork submitted to be sent to panel members and the school I am appealing for a place. I confirm that I have uploaded two household bills with proof of address dated within the last three months. e.g. Council Tax, bank statement, rent book, land line telephone bill, or water bill. The panel reserve the right to conclude an appeal without a decision if these terms are breached.
